Cats - it's sort of a fandom

For the next two years she'll be posted to a hospital in Gabon (she's an Ob/Gyn), but her best friend and roommate for the past eight years has been her large cat, Maximus. He's 9, is 20 inches long, weights 14 pounds (not fat, just BIG), a rescue, and sits right in the center of her heart. Friends who have lived in Gabon for years say that taking him will help with her mental health as she lives and works in such a difficult environment.
Here's the thing: flying for a very long time with a big cat. We're worried about him. About finding a carrier that the airline will approve but that's big enough for him to turn around in during travel. Worried about his mental health. I've never traveled on an airplane with a pet, but I'm hoping some of my fandom friends might have some insight.
The flight from the US to Ethiopia is loooong, then there's a layover, then another flight from Ethiopia to Gabon. Should he wear a harness so she can take him out of the carrier between flights? Will she be allowed to do that? What about pee? Can you use a calming drug or shouldn't you?
Any help or advice would be so welcome. We want to make this trip as comfortable for both of them that we can. Thank you, fandom!
